چکیده

background: accurate and rapid assessment of allograft function is essential in renal transplant recipients in order to detect allograft rejection and to monitor drug nephrotoxicity. we aimed to evaluate the usefulness of cystatin c as a marker of kidney allograft function in the early post-transplant period and to compare this value with that of conventional serum creatinine concentration.   methods: twenty four patients scheduled for kidney transplantation at the kidney transplant center of ghaem hospital, mashhad, iran from september 2006 to november 2007, were sequentially enrolled into the present study. serum creatinine and cystatin c concentrations and urine output were measured daily after transplantation for 3 weeks or until discharge from the hospital.   results: on the 3 rd postoperative day, with a cut-off value of 75 ml/min for glomerular filtration rate, areas under the receiver operating characteristic (roc) curves were 0.926 for creatinine (p=0.021) and 0.815 for cystatin c (p=0.088). at this point creatinine was more sensitive and specific than cystatin c in estimating glomerular filtration rate. on the 7 th day after transplantation, areas under roc curves were 0. 893 for creatinine (p=0.066) and 1.000 for cystatin c (p=0.017). therefore, cystatin c was more sensitive and specific than creatinine in estimating glomerular filtration rate. in two patients with acute rejection and arterial thrombosis, serum cystatin c concentrations increased earlier than serum creatinine.   conclusion: there is a correlation between creatinine and cystatin c early after kidney transplantation. serum creatinine levels seem to be more sensitive and specific for detecting transitory changes in renal function in the 1 st week after transplantation. after the 1 st week after transplantation, cystatin c was more sensitive and specific than serum creatinine concentration.

NINE CASES OF MALIGNANCY AFTER KIDNEY TRANSPLANTATION

Renal transplantation is considered one of the best long-term therapies for chronic renal failure, but unfortunately the incidence of malignancy significantly increases in renal transplant recipients. We analyzed nine cases of malignancy in 200 renal transplant recipients occurring in a 12-year follow-up in Imam hospital of Tehran University, Tehran, Iran. According to this study, the organ...

Serum Cystatin C can detect impaired graft function early after renal transplantation.

OBJECTIVES To study the significance of Cystatin C (Cys C) in early detection of the graft function after renal transplantation. MATERIAL The concentrations of Cys C, Blood urea nitrogen (BUN), Serum creatinine (SCr), and glomerular filtration rate (GFR) were measured. According to the results of the post-transplantation GFR; Seventy renal post-transplanted patients were assigned into 3 group...

Early Acute Kidney Injury based on Serum Creatinine or Cystatin C in Intensive Care Unit after Major Trauma

Background: Acute kidney injury (AKI) is a common problem in critically ill patients and is independently associated with increased morbidity and mortality. Recently, serum cystatin C has been shown to be superior to creatinine in early detection of renal function impairment. We compared estimated GFR based on serum cystatin C with estimated GFR based on serum creatinine for early detection of ...

Cystatin C and renal function in pediatric kidney transplant recipients.

In clinical practice, the glomerular filtration rate (GFR) is often determined with serum creatinine. However, studies have shown cystatin C to be a better parameter for the diagnosis of impaired renal function. We compared GFR estimated by plasma cystatin C with GFR estimated by serum creatinine in a sample of 50 pediatric renal transplant recipients and 24 healthy children. The correlation be...
